Outreach & Engagement Referral Coordinator (This position is hybrid, in-office and remote)

We are seeking an Outreach & Engagement Referral Coordinator to join our team of dedicated and motivated professionals. As the Outreach & Engagement Referral Coordinator you will schedule clients for services in clinical programs and provide targeted outreach to youth, young adults, and caregivers in the outpatient programs, who are struggling to engage. Utilizing scheduling, outreach and case management techniques, you'll support clients in understanding and accessing appropriate treatment and health promotion resources.

RESPONSIBILITIES INCLUDE:

  • Provide a consistent environment of support for agency admissions and intakes.
  • Effectively collaborate with all departments to provide potential clients with easy access to agency services.
  • Develop and implement strategies, along with multi-disciplinary team members, to provide outreach services to hard-to-reach youth, young adults, and caregivers currently enrolled in one of our outpatient programs.
  • Develop knowledge in peer support, community resources, education, housing and food resources aimed at helping clients connect to resources related to managing physical and behavioral health issues.
  • Document all encounters with clients and beneficiaries within appropriate Electronic Health record system, adhering to requirements of MCO’s and HCA.
  • Maintain appropriate personal and professional boundaries with clients, families, co-workers and community resources

If you like problem solving, work well on a team, and have previous administrative or medical office experience, we'd love to hear from you. A Bachelor’s degree is preferred; relevant experience may be substituted.
